Health Services
Wilmington CUSD has a comprehensive health services program for our students and our community. Our quick reference guides below along with community resources and forms at the side of this page will help answer questions you might have about your child at school. Please use the health guidelines when scheduling physicals and immunizations and when determining if your child is too sick for school.

The district nurse provides the following services:

A physical examination is important for preventative, diagnostic, and corrective purposes. Its purpose is to understand and follow up on any health conditions that may adversely affect the student’s well-being and ability to learn.
Illinois immunization regulations specify minimum immunization requirements for school enrollment. The law and regulations provide for the exclusion of students from school if immunizations are not current.
Students are unable to attend school if these requirements are not fully met.
New Students
- Physical Examination (all grades) – parent must complete, sign, and date student health portion at top of page two.
- Proof of immunization dates (month, day, year) as appropriate for grade level
- Dental Examination (grades kindergarten, second, sixth, ninth)
- Eye Examination (kindergarten and out-of-state students entering the IL school system)
Preschool
- Physical examination (fully completed and signed by physician) — parent must complete, sign, and date student health portion at top of page two.
- Lead risk assessment (on physical)
- DTaP (full series) plus a booster on or after the fourth birthday
- Polio (basic series) plus a booster on or after the fourth birthday
- Hepatitis B (series of three)
- Haemophilus influenza type b (Hib)
- Pneumococcal conjugate vaccine
- Measles, Mumps and Rubella (MMR) one dose on or after the first birthday
- Varicella (Chickenpox vaccine) one dose on or after the first birthday
Kindergarten
- Physical examination (fully completed and signed by physician) — parent must complete, sign, and date student health portion at top of page two.
- Lead risk assessment (on physical)
- Body Mass Index (if over 85%-diabetes screening required on physical)
- Dental examination (fully completed and signed by dentist)
- Eye examination (optometrist/ophthalmologist completed and signed)
- DTaP (full series) plus a booster on or after the fourth birthday
- Polio (4 doses) plus a booster on or after the fourth birthday
- Measles, Mumps & Rubella (MMR) completed series (two doses)
- Varicella (chickenpox vaccine) completed series (two doses)
First Grade
- Kindergarten physical examination on file with all required immunizations up to date
Second Grade
- Dental examination (fully completed and signed by dentist)
Third through Fifth Grades
- Kindergarten physical examination on file with all required immunizations up to date
Sixth Grade
- Physical examination (fully completed and signed by physician) — parent must complete, sign, and date student health portion at top of page two.
- Dental examination (fully completed and signed by dentist)
- Body Mass Index (if over 85%-diabetes screening required on physical)
- Tdap vaccine
- Hepatitis B (series of three)
- Meningococcal vaccine (MCV4) one dose on or after eleventh birthday. If your child turns eleven after the first day of school, parent must submit a letter from physician stating when the vaccine will be given.
Seventh through Eighth Grades
- Sixth-grade physical examination on file with all required immunizations up to date
Ninth Grade
- Physical examination (fully completed and signed by physician) — parent must complete, sign and date the student health portion at top of page two.
- Body Mass Index (if over 85%-diabetes screening required on physical)
- Dental examination (fully completed and signed by dentist)
Tenth through Eleventh Grades
- Ninth-grade physical examination on file with all required immunizations up to date
Twelfth Grade
- Meningococcal vaccine (MCV4) second dose on or after sixteenth birthday.
Sports Physicals
- Physical examinations are required for students wishing to try out for interscholastic sports. Sports physicals must be dated within one year of the entire interscholastic season. Physician and parent sections must be completely filled out, signed, and submitted to the child’s school prior to tryouts.
This is an IESA/IHSA requirement.
If a child has asthma, allergies, diabetes, seizures, or any medical condition requiring special health services, please contact the district nurse. The parent/guardian can meet with the nurse and develop an individual health care plan for the student.
There are also links on this page for copies of medical action plans for the child’s doctor to complete and return. If there is an action plan needed that is not linked to the page, please contact the district nurse for assistance.
It is more desirable for parents to administer medication in the home. However, any student who requires any prescription or over-the-counter medication during school hours must comply with the following regulations:
- A physician must complete and sign a Request for Medication Administration form. We must have the form on file in the school office in order for school personnel to comply with the physician's written order. This form is also available for printing in the Forms section.
- We require that medication brought to school be in its original container or in a container properly labeled by the pharmacy or physician. This label must contain the child's name, medication name, dosage, and time your child needs to take the medication.
- All medication must go through the district nurse or the school secretary for proper storage during the day. A doctor's written note must be on file for a student to keep inhalers or epipens with them at school. We do not allow non-prescribed medications in school.
- You must provide the physician's name, address, and telephone number in case we need additional information or in the event that questions arise.
- The district nurse must also approve the above-mentioned form. School personnel should be aware of potential benefits and side effects of the medication used.
- In all cases, the school retains the discretion to reject a request for medication administration depending on individual circumstances.
Wilmington schools require a parent/guardian to complete emergency contact information every year. Parents/guardians must update this vital information whenever changes in contact information occur. There is potential for many emergency circumstances. It is vital that we be able to contact you or your designee at all times.
We require all children to participate in physical education. We understand that in some cases your child cannot be an active participant and will follow the guidelines listed below for these cases.
- When excluding a student from PE participation, we will automatically excuse them from outside recess until released by a physician. This is for the student’s safety.
- We require a parent note and will excuse a child from PE/recess for a maximum of three days.
- For longer exclusion, we require a physician’s note which must state when the child may return to PE/recess.
- We will automatically exclude a student with any injury requiring a cast, crutches, ace wrap, splint, sling, and sutures from PE/recess. This includes any wrap, splint, etc. applied by the parent.
- We require a physician’s note for any crutches, wheelchair or splints, etc. in school. We also require a note if a student will need to use the elevator at the middle or high schools.
- Participation in PE/recess can resume with a note from the physician.
We assess every injury presented to the office. In minor incidents such as bumps, scrapes, etc., the injury is treated and the student returns to the classroom. We will contact parents at the discretion of the district nurse/designee depending on the severity of the injury. We assess serious injuries, give immediate first aid, stabilize the student, and call 911, followed by parent/guardian contact. It is important that the school have on file all contact numbers for a parent/guardian or a responsible person so that the district nurse or principal can reach them, if necessary.
We will send home any child who is assessed in the office with the following symptoms:
- Vomiting
- Diarrhea
- Temperature of 100 or above
- Suspected or confirmed communicable disease
- Other symptoms upon discussion with parent/guardian
Your should keep your child home if he/she has:
- Vomiting/diarrhea within the last 24 hours. Your child must be free of these symptoms without the use of medication for 24 hours before returning to school.
- Fever of 100 or above within the last 24 hours. Your child must be fever-free without the use of medication for 24 hours before returning to school.
- A communicable disease. Your child may return to school after antibiotic therapy for 24 hours.
- An upper respiratory illness that has significant coughing and/or nasal discharge.
The district nurse is always available for consultation. Please do not hesitate to call for any question or concern. You should specifically call for the following reasons:
- New medication or changes in current medication
- New medical diagnosis or a change in health status
- Serious illness, injury, or hospitalization
- Fracture, sprain, cast, or sutures
- Diagnosis of a contagious disease such as strep throat, conjunctivitis (pinkeye), pertussis (whooping cough), chicken pox, or influenza
- Changes within the family that may impact the student while at school
